Please download the following forms relevant to your business, and return to Bradley Accounting Limited along with supporting papers for the year ended March 2024. We are required by the Chartered Accountants Australia and New Zealand to have a signed questionnaire for all financial statements and tax returns we prepare.

All questionnaires are available as either a Word Document which you can complete without needing to print it out or as a PDF.

Individual questionnaire

Everyone needs fill this form in, alongside your relevant business questionnaires.

Business questionnaireS

Use these forms if you are a business owner who traded either as a company, sole trader or partnership. Choose the version you need depending if you want Bradley Accounting to do your GST for you, vs do it yourself or no GST to apply. Don’t forget to fill in your individual questionnaire also.

Rental Questionnaire

Please complete a questionnaire for each house you receive rents from. Don’t forget to fill in your individual questionnaire also.

 

TRUST questionnaires

Use this form if you are trading as a trust. hoose the version you need depending if you want Bradley Accounting to do your GST for you, vs do it yourself or no GST to apply. Don’t forget to fill in your individual questionnaire also.

FARMERS questionnaireS

Use this form if you are a farming business, and you want Bradley Accounting to do your GST.  Supply this form with either the two livestock forms or thoroughbred form AND the completed individual questionnaire.

NZ TAX RESIDENCY

If you have not lived in NZ for all of the last two years then please complete this form.

 

SOLE TRADER questionnaires

Use this form if you are a sole trade. choose the version you need depending if you want Bradley Accounting to do your GST for you, vs do it yourself or no GST to apply. Don’t forget to fill in your individual questionnaire also.
